NEW PATIENT PROCEDURES

 

We look forward to meeting you! Before your initial visit, please download the forms below and bring the completed forms with you to your appointment. If you have had recent blood work or other diagnostic tests done, please bring a copy of those results with you. If you have any questions, please feel free to reach out to us. 

 

WHAT TO EXPECT

 

Please wear loose, comfortable clothing for your appointment, preferably that which can easily be rolled to above the elbows and knees. It is not recommended to fast before your appointment. Make sure to eat a light meal 1 to 2 hours prior to your visit, and refrain from caffeine, nicotine, and alcohol before treatment.  We also recommend that you avoid strenuous exercise shortly before and after treatment.

 

We take the time to get to know each of our patients.  The extended time we spend with you ensures that you get the quality care you deserve.  To that end, our treatments are conducted as follows:

 

Initial Visit:  1.5 hours

 

  • Detailed Medical History

  • Tongue Evaluation

  • Pulse Diagnostics

  • Plan for course of treatment

  • Acupuncture, Cupping, or Gua Sha treatment

  • Herbal Recommendations as appropriate

  • Educational materials

 

Follow-Up Visits:  45 min. - 1 hour â€‹

 

  • Tongue Evaluation

  • Pulse Diagnostics

  • Acupuncture, Cupping, or Gua Sha treatment

  • Herbal Recommendations as appropriate

HERBAL FORMULAS

 

Our office is pleased to offer herbal formulas to our patients in capsule, granule, and tincture form. These formulas are created from professional grade herbs, in accordance with the highest GMP industry standards. Pursuant to FDA regulations and legal guidelines, our office does not accept returns on herbal formulas, including sealed packages.

 

CANCELLATION POLICY  

 

Our office maintains a 24-hour cancellation policy.  Cancellations less than 24 hours in advance and missed appointments are subject to a cancellation fee.  If you are more than 15 minutes late to your appointment, your treatment time may be shortened or your appointment rescheduled.  In case of the latter, the cancellation policy will apply.  Please help us serve you better by keeping your regular scheduled appointment.

 

PAYMENT

 

Payment in full is expected at the time of service. Co-pay and deductibles are due at the time of service. Cash, personal checks, and major credit cards (Visa, Mastercard, Discover, American Express) are accepted forms of payment.
